Recipient Approval Form

This form is for the use of the Emergency Support Sub Committee.

Upon approval of a recipient, the below form is to be completed. In pressing submit, the completed form will be automatically sent to the President, Treasurer, Secretary and the full Emergency Support Sub-Committee. A copy of the completed form will then be tabled at the next monthly full Committee Meeting.
